Résumé
« Comment fonctionne notre collaboration ? » ; « Comment est vécue la transition entre nos institutions par les personnes souffrant de troubles psychiques ? » ; « Que doit-on améliorer ? » Ce sont ces questions que se sont posées la Fondation de Nant, responsable d’organiser les soins psychiatriques dans le secteur est-vaudois, et le Groupe d’accueil et d’action psychiatrique – GRAAP-Fondation, œuvrant pour la réinsertion sociale et professionnelle, au moment où émergeaient pour chacune de ces institutions des projets de déménagement et de réorganisation. Ce questionnement a donné lieu à une recherche-intervention à partir d’entretiens réalisés avec des personnes ayant été suivies dans les deux institutions. D’autre part, de nombreux projets de renforcement de la collaboration ont été formulés ou ont vu le jour. Cet article présente les réflexions qui ont mené à la recherche, ses résultats ainsi que les projets en cours pour améliorer la collaboration entre une structure sanitaire et une structure sociale.
“How will our collaboration operate ?” “How is the transition between our two institutions experienced by patients that have mental health issues ?” “What needs improving ?” These are some of the questions raised by two Swiss structures : the Nant Foundation, responsible for organizing psychiatric care in the southern Vaud area and the Groupe d’Accueil et d’Action Psychiatrique-Fondation (Assessment and Psychiatric Outreach Services Group Foundation), active in social and professional rehabilitation. At a time when both establishments were in the process of moving premises and restructuring their organisations, they developed a close working relationship based on self-assessment and mutual feedback. They also carried out active research based on interviews with patients who had received regular care and monitoring in both of these two institutions. In addition, several other support programmes were set up and rolled out. This article goes on to describe group projects aimed at strengthening the collaborations that were set up and/or implemented. It describes the considerations that led to the research taking place, the results obtained, as well as the projects underway to improve the working relationships between a health and a social care structure.
